  • Department of Public Welfare licensing office of the state in which the home is located
  • County of location
  • Name of home
  • Address
  • City
  • State
  • Zip code
  • Licensed capacity
  • Phone number of home
  • Business status (whether it is a profit or non-profit business)
  • Current type of license that the home's legal entity/ licensee holds for the home (it can be either full/regular license, which is usually 12 months in length, or Provisional Status, which usually is a six-month license)
  • Current status of the license (this indicates whether the license is in good standing with the Department). If you have any questions, you should contact a regional licensing office.
