Personal Care Homes (PCHs) are residences that provide shelter, meals, supervision and assistance with personal care tasks, typically for older people, or people with physical, behavioral health, or cognitive disabilities who are unable to care for themselves but do not need nursing home or medical care. Available services vary and are based on the individual needs of each resident.

• Licensed capacity
• Phone number of home
• Business status (whether it is a profit or non-profit business)
• Current type of license that the home's legal entity/ licensee holds for the home (it can be either full/regular license, which is usually 12 months in length, or Provisional Status, which usually is a six-month license)
